VytlOne’s IMPACT® program was created to deliver the multidisciplinary approach to patient care that’s needed to make a real difference in readmission rates. We use an on-site clinical pharmacist, working in conjunction with your hospital’s nurses, to target patients at high risk of readmission.

Program components

Patient Education

The Impact® program starts with a bedside consultation, enrolling patients as soon as they’re identified as High Risk. At the time of discharge, patients’ medications are delivered, and our clinical pharmacist conducts a comprehensive medication reconciliation with them and/or their caregivers — reviewing all their medications, and explaining the prescription protocols they need to follow.

This ensures that patients leave with both the medication and the knowledge they need to support a successful transition from hospital to home.

Post-Discharge Patient Follow-Up

Once patients are at home, an Impact® Care Coordinator / nurse will reach out to them at least four times over the course of their first month, post-discharge; even more, if their situation warrants additional contact. When patients aren’t maintaining compliance, we’ll work with them to determine Next Steps to solve the problem — whether it’s dosage confusion, inability to pay, lack of transportation, or even unanticipated interactions with existing medication protocols.

Investigating Social Determinants Of Readmission

For starters, we always ask patients if they have transportation. Lack of access can be a huge contributor to a health system’s readmissions — many of which occur simply because patients can’t get to their doctor’s appointments. For those patients, we often provide rides using Uber Health — which typically costs $12 – $30 per patient. Many other patients are readmitted to the hospital because they simply cannot afford their prescriptions. VytlOne also offers a variety of Prescription Payment Assistance options.

Integrating readmission reduction into the health system

Using VytlOne’s proprietary platform, we upload all the data we collect, including patient communications, into our secure database — which our clinical pharmacist can access if any issues arise. When issues are pharmacy related, the clinical pharmacist addresses them. When issues are case-management related, we forward that information to the hospital’s team — then follow alongside them through the completion of each patient’s 30-day program.

Proven results

VytlOne launched its first Impact® program in November, 2023, at South Carolina’s Conway Medical Center. After 15 months, readmissions among patients with three or more prior admissions had dropped 77% — while readmissions among patients with Uncontrolled A1C dropped 93% — resulting in an estimated savings of $1.25 million for CMC.
