By Kim Tzoumakas, CEO, VytlOne

Overview

Health system pharmacy is one of the few growth levers a nonprofit or safety-net hospital fully controls, no new service line, payer negotiation, or certificate of need required. The opportunity is capture: using AI to give finance and pharmacy the same real-time visibility, so a hospital claims the full 340B savings it is already entitled to and turns them into mission capital. Most systems leave meaningful savings unclaimed, not through negligence, but because the rules have outgrown manual processes.

Pharmacy’s role

Ask most health system CEOs what their pharmacy does for the organization, and you’ll get some version of the same answer: it fills prescriptions, it manages inventory, it’s a cost center you keep an eye on. That answer is out of date. Health system pharmacies are one of the few growth levers a nonprofit or safety-net hospital fully controls. No new service line, no payer negotiation, no certificate of need required. Just the discipline to capture what you’re already entitled to under a program designed to support your mission.

It’s worth saying plainly: Congress created 340B so hospitals serving vulnerable communities could turn drug pricing into mission capital rather than having it all retained by manufacturers. That’s not a loophole and it’s not a gray area. It’s the design. When a hospital captures its full 340B savings, it’s using the program exactly as intended: funding charity care, keeping community clinics open, reinvesting in the services that make the mission possible. The systems doing this well aren’t taking advantage of the program. They’re doing their job.

The real story isn’t entitlement, it’s capture

In hundreds of gap analyses, we’ve found meaningful uncaptured 340B savings in nearly every hospital we have partnered with to support pharmacy optimization. That is not because anyone dropped the ball. It’s because eligibility screening, manufacturer restrictions, contract pharmacy rules, and split billing have gotten more complex than manual processes were ever built to handle.

Specialty pharmacy is where the gap runs deepest. Oncology, rare disease, and transplant patients are exactly the ones most likely to slip through manual screening, and a well-run specialty program at a 340B hospital can generate several times the revenue per prescription of a retail program.

Why the 2027 rebate model change raises the stakes

The 340B rebate model change scheduled to take effect on January 1, 2027 is arguably the most consequential shift to hit nonprofit pharmacy economics since the Affordable Care Act. In a lot of systems, the pharmacy leaders and the CFO still haven’t sat down to talk through what it means for their margin or how to monitor the manufacturers and support their pharmacy teams who are already overworked. This is an expensive impending impact with significant complexity and manufacturers who are motivated to pay less to leave unaddressed. Hospital groups including the American Hospital Association have raised concerns about the model’s cash-flow and administrative burden on covered entities.

The chief pharmacy officer sees the gap first

Chief pharmacy officers are usually the first ones to sense the gap. They just don’t always have the data, the platform, or the resources to prove the size of it and make the case for investment. The CPOs who are getting in front of this become some of the most valuable strategic partners a CEO and CFO have, protecting the integrity of the program and keeping the dollars associated with it inside the system. The best-run 340B programs I’ve seen have one thing in common: the CEO, the CFO, and the pharmacy leaders are looking at the same numbers, in the same room. That alignment is rarer than it should be, and when it happens, the results aren’t incremental.

How AI closes the gap between finance and pharmacy

This is the gap VytlAIQ was built to close, giving finance and pharmacy the same real-time visibility so they’re planning from shared facts instead of competing hunches. It doesn’t replace the pharmacy team. In fact, the team is critical to its success. It gives them the speed and evidence to stay ahead of a program that keeps getting more complex, not less.

The question worth asking

Your pharmacy program is either generating margin that funds your mission, or it’s quietly limiting it. There’s not much of a middle ground. I can usually tell a health system leader, within a conversation, roughly what their pharmacy should be capturing versus what it likely is, and that gap tends to open up a more hopeful conversation than people expect, because it’s not a problem you have to solve by cutting something. It’s capital you’re already entitled to and simply haven’t claimed yet. The 2027 rebate model puts even the best-optimized systems at risk of losing significant dollars, or at a minimum, facing significant delays in payment.

The systems that lean into this now, pharmacy and finance in the same room looking at the same data, are the ones that will have room to grow when everyone else is still debating whether the opportunity or the risk is real. If you want to see what that looks like in practice, talk with our team.

Key takeaways

Health system pharmacy is one of the few growth levers a hospital fully controls, and 340B savings are mission capital it is already entitled to.

Most systems leave meaningful savings unclaimed because eligibility, manufacturer restrictions, and split billing have outgrown manual processes, with specialty pharmacy the deepest gap.

The January 2027 rebate model change raises the stakes for nonprofit pharmacy economics.

AI closes the gap by giving finance and pharmacy the same real-time visibility, so they plan from shared facts. It supports the pharmacy team, it does not replace it.

Frequently asked questions

Why should a hospital treat pharmacy as a strategic asset?

Because it is one of the few growth levers a nonprofit or safety-net hospital fully controls. Capturing the full 340B savings a hospital is entitled to requires no new service line, payer negotiation, or certificate of need. Those savings become mission capital that funds charity care, community clinics, and the services that make the mission possible.

What is 340B mission capital?

It is the savings a hospital captures under the 340B program, used as Congress intended: to fund care for vulnerable communities. When a hospital captures its full 340B savings, it is using the program exactly as designed, turning drug pricing into support for its mission rather than leaving that value with manufacturers.

How does AI help a health system capture more 340B value?

AI gives finance and pharmacy the same real-time visibility, so they plan from shared facts instead of competing estimates. It helps teams keep pace with eligibility screening, manufacturer restrictions, contract pharmacy rules, and split billing that have grown too complex for manual processes. It supports the pharmacy team rather than replacing it.

How does the 2027 rebate model change affect nonprofit hospitals?

The 340B rebate model change taking effect in January 2027 is a significant shift for nonprofit pharmacy economics. It puts even well-optimized systems at risk of losing dollars or facing payment delays, which is why finance and pharmacy leaders should align on their exposure now rather than after it takes effect.

How VytlAIQ turns pharmacy data into decisions

By Heather Brooks, Vice President, Pharmacy Services Products, VytlOne

How does AI improve pharmacy analytics for health systems? AI connects the data scattered across a health system’s pharmacy programs, from EMRs, third-party administrators, dispensing systems, and patient management platforms to 340B platforms, and normalizes it into one continuously updated view. That lets leaders see performance in real time, catch problems early, spot missed 340B and specialty opportunities, and make strategic decisions from clean data instead of manual reports. VytlAIQ is VytlOne’s AI-powered intelligence platform built to do exactly this for 340B, outpatient retail, specialty, and clinic-owned pharmacy programs.

Health system leaders overseeing 340B programs and outpatient pharmacy operations are managing some of the most complex, data-intensive work in healthcare. They are responsible for seeing that 340B savings flow back to the communities they serve, that outpatient retail and specialty pharmacies are retaining patients, and that clinic-owned pharmacy programs are reaching their full potential. The challenge has never been the availability of data. It has always been the ability to make sense of it quickly enough to act. VytlAIQ gives leaders real-time analytics, predictive insights, and decision-support tools built for exactly that complexity.

The five ways at a glance

AI unifies data from every pharmacy system into one real-time view.

AI catches performance signals early, while they are still easy to fix.

AI surfaces missed 340B, retail, and specialty opportunities.

AI flags patient leakage before a patient is lost.

AI supports strategic decisions with clean, interpreted data.

1. AI brings all your pharmacy program data into one platform

Meaningful insight starts with complete data, and for health system pharmacy programs, that data lives in many places at once. A common frustration among pharmacy leaders is how long it takes to get a clear, reliable picture of program performance. Reports are often built manually, arrive on a lag, and require significant interpretation before they can inform a decision. Electronic medical records, third-party administrators, pharmacy dispensing systems, patient management platforms, and 340B platforms each hold a piece of the picture. Without a way to connect them, leaders make decisions on partial information or spend hours manually pulling data before analysis can begin.

VytlAIQ was built to solve that at the foundation. The platform ingests data from all of these source systems, normalizes it, and presents it through a single access point designed for health system pharmacy leadership. Rather than navigating multiple reports from multiple systems, leaders log into one platform and see a unified view of how their 340B, outpatient retail, and specialty pharmacy programs are performing. Because the data is consolidated and continuously updated, the dashboards reflect what is happening now, not what was happening when the last report ran. When something needs attention, it is visible. When programs are on track, that is visible too. The result is a leadership team that spends less time gathering data and more time acting on it.

2. AI turns data signals into early action

In health system pharmacy, the difference between a manageable issue and a major operational problem often comes down to timing. By the time a downward trend appears in a monthly report, the window for easy correction has usually passed. VytlAIQ closes that gap by identifying signals in the data early, while there is still time to act without disruption.

The platform continuously monitors performance indicators across all pharmacy program types, looking for patterns that precede problems rather than documenting them after the fact. A gradual decline in 340B-eligible prescription capture, a slowdown in specialty therapy starts, or a dip in refill adherence at a clinic-owned location are all conditions VytlAIQ can surface before they register as a trend leadership has to explain. When those signals appear, the platform brings them forward with enough context to understand what is happening, where, and what options exist to address it.

That early visibility changes how leadership teams operate. Rather than reacting to performance problems after the fact, they are positioned to make small, targeted adjustments that keep programs on track. Over time, that shift from reactive to proactive management has a meaningful impact on program performance, patient retention, and the financial health of the programs the health system depends on.

3. AI surfaces pharmacy program opportunities you’re missing

Health systems often do not know what they are missing until the data is organized to show them. VytlAIQ uses predictive analytics to identify underutilized opportunities across all three program types: 340B-eligible prescriptions that are not being captured, patients who miss the chance to fill at the health system retail pharmacy, and specialty therapy candidates being referred out of network.

For example, a health system may have a strong oncology program but route specialty patients to an external partner by default rather than capturing them in its own specialty pharmacy. VytlAIQ can identify that pattern, quantify the financial and clinical impact, and help leadership make the case for an operational change. These are not hypothetical opportunities. They are real gaps that exist in most health system pharmacy programs and are difficult to see without the right analytical lens.

4. AI spots patient leakage before it becomes a loss

Patient leakage occurs when a patient fills a prescription outside the health system’s pharmacy network. It is one of the most persistent and costly challenges pharmacy leaders face, and historically, identifying it required manual data pulls and retrospective analysis that arrived too late to act on.

VytlAIQ changes that by continuously analyzing patient prescription activity across 340B, outpatient retail, and specialty programs, identifying patterns that signal leakage risk before a patient is lost. For example, a patient discharged from a health system clinic who has not filled a follow-up prescription within an expected window can be flagged automatically, giving the care and pharmacy team a chance to re-engage. For 340B programs specifically, every prescription that leaves the network is a missed opportunity to capture savings that fund critical patient services.

5. AI supports better decisions about your pharmacy programs’ future

The most valuable use of AI in health system pharmacy is not describing what has happened. It is helping leadership decide what comes next. VytlAIQ combines historical performance data, predictive modeling, and the ability to ask the platform a direct question, giving leaders the context to think strategically about their programs. For 340B leaders, that means assessment and planning supported by visibility into program compliance, savings generation, eligible prescription capture rates, and change risk modeling. For outpatient retail and specialty programs, it means fill rates, patient leakage, adherence benchmarks, and revenue performance, plus the unique attributes affecting each.

Whether a health system is evaluating whether to expand its retail pharmacy reach, change its 340B program structure, or invest in specialty pharmacy growth, those decisions benefit from clean, organized, AI-interpreted data. VytlAIQ is designed to meet health system leaders wherever they are, whether managing day-to-day operations or planning three years out.

The future of health system pharmacy is one where AI handles the complexity of data aggregation and pattern recognition, freeing health system, pharmacy, and operational leaders to focus on what only people can do: building relationships, driving strategy, and delivering better outcomes for patients.

Frequently asked questions

What is pharmacy analytics?

Pharmacy analytics is the practice of collecting and interpreting data across a health system’s pharmacy programs, including 340B, outpatient retail, specialty, and clinic-owned, to measure performance, catch risks early, and guide decisions. AI platforms like VytlAIQ consolidate that data into one real-time view.

How does AI help manage 340B programs?

AI connects 340B platform data with dispensing and patient records to track eligible prescription capture, savings generation, and compliance, and to flag leakage and missed capture before savings are lost.

What is patient leakage in health system pharmacy?

Patient leakage is when a patient fills a prescription outside the health system’s own pharmacy network. AI can flag leakage risk early, for example when a discharged patient has not filled a follow-up prescription, so teams can re-engage.

What is VytlAIQ?

VytlAIQ is VytlOne’s AI-powered pharmacy intelligence platform for health system leadership. It unifies data across 340B, outpatient retail, specialty, and clinic-owned pharmacy programs to deliver real-time analytics, predictive insights, and decision support.

See VytlAIQ in action

Health system pharmacy programs are sitting on data that could be driving better decisions today.
Why AI Won’t Replace Pharmacists

By Heather Brooks, Vice President, Pharmacy Services Products, VytlOne

Will AI replace pharmacists? No. In health system pharmacy, AI is augmenting the team, not replacing it. It takes on documentation, data gathering, and routine checks, while pharmacists, pharmacy technicians, and Prescription Access Liaisons (PALs) keep authority over clinical decisions, patient counseling, and care. What staff working alongside these tools describe is not replacement but relief: relief from documentation overload, from hours on hold with insurance companies, and from the mental weight of catching errors in a high-volume, high-stakes environment.

The concern is understandable. AI is moving fast, the headlines are loud, and no one wants to feel like their expertise is being automated away. But the professionals who work with AI day to day are not being pushed out. They are being freed up to do the work only a trained, caring human can do. Here are five ways AI is making that difference.

The five ways at a glance

AI streamlines the prior authorization process for pharmacy staff.

AI adds a second layer of error checking in high-volume dispensing.

AI puts complete medication history at every role’s fingertips.

AI supports clinical decisions with real-time evidence.

AI reduces administrative burden so staff can focus on patients.

1. AI streamlines the prior authorization process for pharmacy staff

Prior authorizations (PAs) are one of the most time-consuming, frustrating parts of pharmacy work, and one of the most consequential for patients. A delayed PA can mean a delayed therapy start, a gap in critical medication, or a patient who walks away from care they need. This is especially true in health system specialty pharmacy, where PA volume is high. AI is changing the pace by pulling a patient’s medication history, diagnosis codes, clinical documentation, and payer-specific criteria into one place. That consolidation helps PALs, pharmacists, and pharmacy techs build stronger, faster PA submissions without the hours-long hunt across multiple systems.

How this works in practice

A patient arrives to pick up a newly prescribed specialty medication and learns their insurance requires a prior authorization before it can be dispensed. The PAL opens the workflow, and within minutes AI has surfaced the patient’s relevant diagnosis history, current medication list, prior therapy attempts, and the specific clinical criteria the payer requires, all without a phone call to track down records. The pharmacist reviewing the case uses that same medication history to confirm the new therapy is appropriate given what the patient already takes. The PA is submitted the same day. The patient, who expected a days-long wait, gets a call that afternoon with an update and leaves trusting the pharmacy team is in their corner.

2. AI reduces errors in high-volume dispensing

The dispensing environment in health system retail and specialty pharmacy is fast-moving and unforgiving. Medication errors, even small ones, can have serious consequences, and the cognitive load on dispensing pharmacists and pharmacy techs is significant. AI-assisted verification tools give those teams an additional layer of review that does not get tired, distracted, or have a bad day. By flagging potential drug interactions, dose discrepancies, or duplicate therapies before a medication leaves the pharmacy, AI acts as a reliable second check. The pharmacist’s judgment is not replaced. A supportive backstop helps catch what human eyes sometimes miss.

How this works in practice

A pharmacy tech is processing a high volume of prescriptions during a busy afternoon when a new order comes through for a patient already on a complex regimen. The tech prepares the medication and routes it for pharmacist verification. Before the dispensing pharmacist completes the check, the AI verification system flags a potential interaction between the new prescription and a medication the patient takes for a chronic condition, one filled at a different location that might not have been visible in the active queue. The pharmacist confirms the interaction is clinically significant and contacts the prescribing provider to discuss an alternative. The prescription is updated before it reaches the patient. What could have been a harmful outcome becomes a routine clinical intervention.

3. AI puts complete medication history at every role’s fingertips

A patient’s medication history is foundational to nearly every decision in health system pharmacy, and accessing it has historically been anything but simple. Information lives across systems, visits, and providers, and pulling a complete picture together takes time pharmacy staff often do not have. AI changes this by aggregating and presenting medication history in a structured, accessible format, and the benefit reaches every role on the team. When all roles share the same complete picture, handoffs are smoother, assessments are more thorough, and patients work with a team that knows their history.

How this works in practice

A patient is being discharged with a new prescription added during their inpatient stay. The pharmacist conducting discharge medication reconciliation needs to evaluate whether the new therapy is safe alongside the medications the patient took before admission, prescribed by specialists, a community pharmacy, and a mail-order service. Instead of piecing that history together from multiple portals and phone calls, the AI tool surfaces a consolidated medication list in seconds. The pharmacist identifies a duplication of therapy and resolves it before discharge. The patient goes home with the right medications, the right instructions, and a care team working from the same page.

4. AI clinical decision support surfaces real-time evidence

Pharmacists apply an enormous breadth of knowledge in real time: dosing adjustments based on kidney or liver function, drug interaction management, therapeutic alternatives, and formulary considerations that change with each payer. Staying current with clinical guidelines while managing an active caseload is one of the most demanding parts of the role. AI tools that surface relevant clinical evidence, guideline summaries, and decision-support prompts at the point of care reduce that cognitive load without reducing the pharmacist’s authority. The pharmacist still makes the clinical decisions, now with better, faster access to the information that supports them.

How this works in practice

A pharmacist is consulted on a patient whose new prescription requires a dose adjustment due to reduced kidney function found in recent lab work. The pharmacist needs to determine the appropriate dose, confirm there are no additional interaction concerns given the patient’s other medications, and document the recommendation. The AI clinical decision-support tool surfaces the relevant dosing guidance, flags a secondary interaction that warrants monitoring, and pulls the current guideline supporting the adjustment. The pharmacist reviews, applies clinical judgment, and delivers a recommendation to the provider in a fraction of the usual time. The patient’s therapy starts safely and on time.

5. AI reduces administrative burden so staff can focus on patients

Ask anyone in health system pharmacy what they wish they had more time for, and the answer is almost always the patient. The work that drew most pharmacy professionals to the career, counseling someone on a new medication, advocating for access to a therapy, and building the relationship that makes a patient feel cared for, is also the work most likely to get crowded out by administrative demands. AI reclaims that time by streamlining documentation, triaging routine inquiries, and handling the repetitive workflows that quietly consume hours of the day.

How this works in practice

A patient arrives to pick up a new therapy with important instructions, potential side effects, and follow-up steps to understand before leaving. Normally the dispensing pharmacist is already behind, with prescriptions queued for verification, a phone ringing at the counter, and unfinished documentation from earlier. But because AI has handled documentation consolidation for new therapies and reduced manual research on several drug-drug interactions, the pharmacist’s queue is manageable. The pharmacist has the time and bandwidth to work with the patient and make sure they understand their new medication, building trust and improving adherence.

Where this is heading

Across all five areas, AI is moving from reactive to proactive. Prior authorization is heading toward real-time eligibility checks at the point of prescribing and automated status tracking, turning a days-long process into hours. Verification is moving toward predictive risk modeling and AI-enhanced imaging that catches labeling and selection errors under real-world conditions like glare, poor lighting, and awkward angles. Medication history tools are starting to flag gaps and missing records, not just aggregate what exists. Clinical decision support is becoming proactive, surfacing guideline updates and patients due for review before anyone asks. And documentation is increasingly handled by natural language processing that turns clinical interactions into structured notes with minimal staff input. The through-line is the same: AI takes the work that can be automated so pharmacy staff can spend their time where only a human can.

The bottom line

AI in health system retail and specialty pharmacy will not replace the expertise, judgment, and compassion that pharmacy professionals bring to their work. What it can do is take on the administrative tasks that drain those qualities, the documentation and the data gathering. For pharmacists, PALs, and pharmacy techs, the promise of AI is a more supported team, not a smaller one. It is a team where errors are caught before they reach a patient, prior authorizations move faster, and information is readily available, so patient care stays front and center and the daily administrative frustrations of pharmacy practice are muted.

Frequently asked questions

Will AI replace pharmacists?

No. AI augments pharmacists rather than replacing them. It handles documentation, data gathering, and routine checks, while pharmacists keep authority over clinical decisions, counseling, and patient care.

Will AI replace pharmacy technicians?

No. AI takes on repetitive workflows like record retrieval and verification support, which frees pharmacy technicians and Prescription Access Liaisons for higher-value work rather than eliminating their roles.

Will AI take over pharmacy?

The realistic near-term picture is augmentation, not takeover. AI catches errors, speeds prior authorizations, and reduces administrative load, while trained professionals make the judgment calls patient safety depends on.

How is AI used in health system pharmacy today?

AI is used to streamline prior authorizations, add a second layer of error checking in dispensing, consolidate medication history, surface clinical evidence at the point of care, and cut administrative burden so staff can focus on patients.

See how VytlAIQ supports health system pharmacy teams.

Request a demo.

Heather Brooks is a pharmacist and the Vice President of Pharmacy Services Products at VytlOne.
By Heather Brooks, Vice President, Pharmacy Services Products, VytlOne

How is AI improving the specialty pharmacy patient journey? AI is helping health system specialty pharmacies make the medication journey simpler, faster, safer, and more affordable, improving medication access at every step. It flags affordability barriers early, speeds prior authorizations, closes refill gaps, coordinates care across clinic and payer, and surfaces safety risks sooner, with pharmacists and care teams reviewing every step. The strongest results come from AI working alongside integrated clinic and pharmacy care teams and the patient.

In health system outpatient retail and specialty pharmacy, the patient journey includes coverage checks, prior authorizations (PAs), medication therapy management, refills, delivery, and human support. Below are five ways AI is improving that journey.

The five ways at a glance

Affordability support becomes proactive instead of reactive.

Prior authorization moves from fax-and-wait to a guided, faster submission.

Refills and reminders close adherence gaps before they form.

Care feels coordinated across clinic, pharmacy, and payer.

Safety risks are flagged earlier, with humans in the loop.

1. AI makes medication affordability proactive

One of the biggest patient stressors is cost uncertainty, particularly for specialty therapies where copays can be high and assistance programs are complex. That same specialty complexity is where the 340B revenue most health systems miss tends to concentrate. In strong health system models, affordability support is already core to the care experience: identifying copay assistance, coordinating financial support, and setting expectations early. AI makes that support faster and more consistent by flagging likely affordability barriers, prioritizing who needs help first, and prompting the right assistance pathways at the right moment, before a patient abandons therapy.

How this works in practice

Marisol arrives at the pharmacy counter expecting a routine pickup. The initial copay is far higher than she expected. Instead of being told to check back, the pharmacy has proactively come up with two options: an assistance pathway and a clinically appropriate alternative to discuss with the clinic. The result is not that AI saved the day, but less panic, fewer delays, and a plan that supports her access to medication.

2. AI speeds prior authorization in specialty pharmacy

For specialty pharmacy patients, delays in prior authorization mean delays in therapy. PAs can take several days and sometimes longer, which is why time to therapy shows up as a top metric in specialty pharmacy performance. AI helps by drafting payer questionnaire responses, extracting relevant clinical evidence, and reducing the time staff spend rekeying information. AI can reduce work time per authorization from as much as 25 minutes to about five minutes, and submissions can go in about a day earlier.

How this works in practice

Darryl is prescribed a new, high-cost injectable after a flare-up. Historically, starting this drug takes weeks. VytlOne’s Prescription Access Liaisons can use AI-assisted drafting of the PA, shortening the time between prescribed and in hand. The pharmacist reviews for accuracy, and the request goes out sooner. The patient sees the care plan moving forward instead of stuck in paperwork.

3. AI closes refill gaps and improves medication adherence

Nonadherence often is not dramatic. It is small gaps: a refill missed during a busy week, a pickup delayed, a message forgotten. Digital reminders and nudges, especially text-based ones, are patient-friendly, scalable ways to reduce refill gaps, one of several medication adherence solutions AI makes smarter. AI makes this smarter by timing outreach based on behavior patterns, channel preference, and risk signals rather than a fixed day 28 text, and it does so without burying pharmacy staff in manual calls.

How this works in practice

Sam manages diabetes medications, blood pressure medications, and a statin, along with work and family. An AI-supported refill program notices a refill gap forming, sends a gentle nudge, and offers a quick deliver-or-pickup option. Sam stays on track without spending 30 minutes on hold.

4. AI coordinates care across clinic, pharmacy, and payer

Patients often experience the system as fragmented: the clinic says one thing, the payer says another, and the pharmacy is left translating. VytlOne’s health system specialty pharmacy models emphasize coordination by embedding Prescription Access Liaisons in provider offices, which allows for consistent updates and fewer dropped handoffs. AI improves coordination by acting like a workflow air traffic controller: routing tasks, flagging missing information, and keeping statuses visible so the next best action is obvious to the team, the same shift toward action that AI-driven pharmacy analytics brings to health system leadership. That visibility and orchestration turns a chaotic journey into a predictable one.

How this works in practice

The prescriber tells Nina the pharmacy will have her prescription ready. Instead of arriving to learn the prescription must be changed to the insurance formulary product, Nina gets predictable updates: benefits verified, PA submitted or alternates discussed with the prescriber, assistance in progress, delivery scheduled. Her journey from prescriber to medication in hand is handled with white-glove service.

5. AI flags medication safety risks earlier, with humans in the loop

Medication safety is not just dispensing accuracy. It also includes duplicate therapies, wrong-dose risks, and missed clinical signals. AI can flag potential problems early, and the critical design point is human review, which allows for proactive discussions among the prescriber, pharmacist, and patient.

How this works in practice

George has a medication change after a hospitalization. The system flags a possible duplication risk with a home medication. The pharmacist confirms with George that he has the duplicate at home, and the final regimen accounts for it while avoiding duplicative therapy. George feels someone is paying attention, and his confidence in his care team rises.

Where this is heading

Across all five areas, AI is shifting specialty pharmacy from reactive to anticipatory. Expect affordability support to move closer to real time, predicting cost barriers before a prescription is finalized. Prior authorization is trending toward proactive readiness, with AI predicting friction and helping teams build the approval packet upfront. Refill support is becoming anticipatory, catching insurance changes and shipment constraints before they become emergencies. Coordination is getting more structured, and safety is moving toward continuous medication surveillance that points pharmacists to the highest risk first.

A better experience, because the basics work better

Health system outpatient retail and specialty pharmacies are already positioned differently: closer to the clinic, connected to the care plan, and built to support medication access end to end. Used well, AI strengthens this model. Cost conversations happen earlier, authorizations move with fewer delays, refills do not slip into gaps, and safety risks are flagged sooner. The patient journey becomes more predictable and supported, and pharmacy teams get more time back for counseling and problem-solving instead of paperwork.

5 Things Pharmacy Leaders Should Learn

By Kim Tzoumakas, CEO, VytlOne

Building AI in health system pharmacy means putting the operational foundation in place first and then using AI to amplify it. In practice, that means AI works best when it augments experienced pharmacy teams rather than replacing them: catching compliance gaps before they become denials, handling repetitive work, and freeing pharmacists and liaisons to spend their judgment where it changes patient outcomes. The technology is rarely the obstacle. Operational clarity is.

Technology is never really the hard part. I wish it were because that would make my job easier. The hard part is sequencing, culture, and building the operational foundation that technology can amplify.

I spent years working inside health systems, early on as an attorney advising on strategy, hearing the same conversations again and again: not enough capital to improve outcomes or provide critical services. The 340B compliance gaps I saw were rarely about bad intentions. They came from programs outpaced by complexity and heavy regulatory change, moving faster than teams could manage. I watched health systems lose money they didn’t have to lose, or take overly conservative stances out of fear of regulatory audits. It stuck with me, and it’s the lens I bring to every leadership decision I make today.

Since then, I’ve had the privilege of leading transformation and growth across three very different multi-state healthcare organizations: oncology, radiology, and now pharmacy. These are different clinical worlds, but the same lesson keeps showing up. The technology is rarely the obstacle. Operational clarity and focus are. Get those right and success tends to follow almost naturally. Skip them and AI just helps you move faster in the wrong direction.

Why 340B complexity makes the operational foundation non-negotiable

The 340B program has grown far more complex. There are manufacturer restrictions, audit exposure, and contract pharmacy scrutiny. All of it lands on teams that are also trying to take care of patients every day, often with limited resources. When I joined VytlOne, I found a pharmacy operational team with real depth. What we didn’t have 18 months ago was one unified platform that could work across all of pharmacy operations with the speed and visibility this moment needs.

Building VytlAIQ ourselves, rather than buying something and bolting it on, was deliberate. What we had at the start was unique and would set us apart: a century of pharmacy history, three decades of managing health system pharmacies, and operational data that is real, clean, and ours.

When a health system leader is skeptical of AI, I understand it, and it usually works in our favor. We did not want to be a technology player chasing short-term wins. Instead, we built a platform to augment teams in hospitals as experienced as the ones inside VytlOne. There is a real difference between AI shaped by people who have actually run 340B workflows and a product built by a technology player.

Where AI transformation stalls in health systems

Most transformation struggles in the space between a confident technology decision and the operational reality underneath it. What I am proudest of with VytlAIQ is not the technology itself. It is what it frees people to do: catch the small things that would otherwise become denials or missed opportunities, and quietly handle the work that used to consume pharmacists’, liaisons’, and compliance teams’ time. It does not replace people. It gives them room to use their judgment where it matters most, which is keeping patients on their medicines and delivering care that technology alone never will.

What I’ve come to believe about AI and pharmacy margin

Culture shapes how well technology lands. The hardest conversations are often with the people in the trenches who do not have the bandwidth to assess how the right resources and partners could strengthen operations they have already made successful. That assessment should never feel like a threat or a sign of failure. It reflects leadership and financial rigor at the moment hospitals need both most.

Pharmacy should be driving significant investment in every hospital through the margin it can generate. Without those dollars, patients suffer and services lag across the healthcare system. VytlAIQ exists to augment and support pharmacy teams so they can generate the margin that lets hospitals reinvest in critical programs.

Key Takeaways

In health system pharmacy, the operational foundation, not the technology, decides whether AI delivers.

340B complexity (manufacturer restrictions, audit exposure, contract pharmacy scrutiny) makes that foundation non-negotiable.

VytlOne built VytlAIQ in house to augment experienced pharmacy teams, not replace them.

Done right, AI in health system pharmacy protects margin, and that margin lets hospitals reinvest in patient care.

FAQs

5 Things Pharmacy Leaders Should Learn

By Joel Wright, President of Pharmacy Services, VytlOne

Artificial intelligence (AI) is moving from pilot projects into the core of pharmacy operations. In health system pharmacy and 340B programs, that shift carries more weight than almost anywhere else, because here AI touches patient access, compliance risk, and the use of limited resources across vulnerable populations.

After working inside these programs, the lesson I keep returning to is simple. The value of AI is not decided by how advanced it is. It is decided by how thoughtfully it is governed and how closely it stays tied to patient outcomes.

Here are five things every pharmacy leader should learn before adopting it.

1. AI in pharmacy is a governance decision, not a technology purchase

Most technology decisions reside within IT. AI in a health system-owned pharmacy extends far beyond technology, enabling smarter patient outreach, more effective resource allocation, and greater visibility into 340B opportunities. As AI becomes a strategic driver of outcomes, governance is what makes sure that innovation is applied responsibly and consistently.

Effective governance includes:

Clear accountability for AI-enabled decision support.

Oversight of any model that influences outreach or prioritization.

Continuous monitoring for differential effects across patient populations and payer types.

Transparency about how recommendations are generated.

The ability for pharmacists and clinicians to override automated guidance.

Before you evaluate a single feature, answer one question. When AI influences a decision, who is accountable for it?

2. Responsible AI should reduce bias and widen access, not narrow it

AI has the power to enhance decision-making and operational performance. With the right governance and design principles, organizations can proactively address potential biases and support more consistent outcomes.

Designed responsibly, AI does the opposite of what people fear. By expanding the data signals it considers, incorporating human oversight, and regularly testing for unintended disparities, it can surface patients who might otherwise be missed, including those who are newly diagnosed or facing barriers like affordability and access. That same visibility is what surfaces the 340B revenue most health systems miss, the eligible cases manual screening skips over. In a pharmacy that sits inside the clinical care continuum, that capability lets an organization extend access more intentionally and strengthen patient trust.

3. The point of AI is action, not better reports

Traditional pharmacy analytics are good at explaining what already happened. Revenue and cost trends, capture rates, adherence percentages. What reporting rarely tells a team is what to do next.

For AI to change that, it has to be grounded in a holistic data strategy. Most of the data that supports pharmacy decisions is scattered across clinical systems, pharmacy platforms, payer feeds, and financial tools, or never captured at all. Connecting those sources, and delivering insight in time to influence care, is the foundation.

The next evolution of pharmacy intelligence surfaces issues while prescriptions and referrals are still upstream, prioritizes intervention based on clinical and operational context, and routes work to the right team members. That move from hindsight to insight is where AI starts to earn its place.

4. Human review is the most important design point

The most important design point in any responsible system is human review. AI can surface insights and opportunities, but clinicians and pharmacy teams remain responsible for every care and operational decision.

Pharmacists and clinicians need a real ability to override automated recommendations, not a checkbox that exists on paper. That is what keeps AI aligned with pharmacy operations, 340B requirements, and patient safety, while still accelerating the work.

5. Where your AI comes from matters

As adoption accelerates, the source of the intelligence matters as much as the intelligence itself. Many of the solutions entering the market today are built by strong engineering teams with no pharmacists, no 340B compliance depth, and no history operating inside the complexity of a health system.

The foundation needs to be built and validated by pharmacists and operators who have actually worked inside the systems the technology intends to serve.

That is the thinking behind VytlAIQ, our AI enabled pharmacy intelligence platform built specifically for health system environments. It is designed around continuous 340B eligibility review, real time performance visibility across pharmacy operations, workflow automation, and next best action guidance to support clinical decision making. The point is not the feature list. The point is that responsible AI shows up as integrated workflows and data models aligned with compliance, care delivery, and equity goals.

Looking ahead: responsible AI and sustainable 340B performance

AI has real potential to help health system pharmacies navigate the growing complexity of 340B, specialty pharmacy, and access management. Its value will not be determined by sophistication alone. It will be determined by how thoughtfully it is governed, how transparently it operates, and how closely it remains tied to patient outcomes.

For organizations willing to lead with accountability and clinical oversight, AI becomes a tool that supports sustainable 340B program optimization while advancing the core mission of equitable, high-quality care.

A VytlOne Member Services Success Story

What is responsible AI in pharmacy?

Responsible AI in pharmacy means using artificial intelligence to support pharmacy teams, not replace them. Trained people stay in the loop on consequential decisions, member data is protected under the Health Insurance Portability and Accountability Act (HIPAA), and the technology’s limits and potential for bias are disclosed openly. The goal is better care, guided by people. That’s part of a broader shift health systems are watching closely: AI governance in healthcare, meaning who reviews an AI system’s decisions, what data it touches, and how vendors disclose its limits.

Summary

VytlOne leverages AI in its contact center to enhance human connection, streamline tasks, and provide member advocates with insights, enabling them to focus on engaging and problem-solving for members.

AI analyzes conversations to identify trends and delivers real-time coaching, resulting in continuous improvement, reduced member abrasion, and high member satisfaction scores.

The company is implementing AI-powered call summarization to further improve efficiency, consistency, and personalized member experiences by freeing staff from manual notetaking.

The impact of AI

At VytlOne, we’re using artificial intelligence (AI) to strengthen the human connection in our contact center. AI enables our highly trained member advocates by streamlining tasks and surfacing insights, which allows our team to focus on what matters most: listening, engaging, and solving problems for our members.

How we’re using AI today

AI transcribes and analyzes calls to identify meaningful conversation “moments,” grouping common topics and inquiries. These insights help us proactively identify trends, deliver real-time coaching, and focus resources where they are needed most. This is one example of responsible AI in pharmacy: the technology handles the pattern-finding, and skilled people handle the care. The results speak for themselves:

4.89 out of 5.0 member satisfaction based on more than 50,000 surveys in 2025.

What’s next?

VytlOne is implementing AI-powered call summarization to free advocates from manual notetaking, increase documentation consistency and quality, and enable faster resolutions. Most importantly, this provides team members additional time for active member connection and personalized, high-touch experiences.

For a closer look at how we govern these tools across our pharmacy programs, read “Responsible AI in Pharmacy: VytlOne’s Approach”

If you’d like to learn more about how VytlOne supports members through innovation and care, reach out to your VytlOne representative.

Introducing VytlAIQ

Have you heard about VytlAIQ, our new end-to-end intelligence platform designed for health system 340B and specialty pharmacy services?

VytlAIQ unifies data, automates workflows, and delivers predictive intelligence to help pharmacists and healthcare providers maximize 340B performance, accelerate patient access, and unlock the full value of specialty and pharmacy services.

Learn more about VytlAIQ or schedule a platform briefing to see how AI supports members through innovation and care.

Frequently asked questions

What is responsible AI in pharmacy?

Responsible AI in pharmacy is the practice of using artificial intelligence to support pharmacy and member-services teams rather than replace them. It keeps trained people in charge of consequential decisions, protects member data under HIPAA, and stays transparent about what the technology can and cannot do, including its potential for bias. At VytlOne, that means AI handles the pattern-finding and routine tasks so member advocates can spend more time listening, problem-solving, and building trust with the people they serve.

How does AI help pharmacy contact centers?

In a pharmacy contact center, AI transcribes and analyzes calls to surface common questions, recurring themes, and coaching opportunities across thousands of interactions. Those insights help teams spot trends early, deliver real-time coaching, and direct resources where members need them most. AI can also summarize calls automatically, which reduces manual notetaking and speeds up follow-up. The result is more consistent documentation and more time for advocates to focus on the member in front of them.

How does AI affect the member experience?

Used responsibly, AI improves the member experience by removing friction, not adding it. By handling transcription, analysis, and summarization behind the scenes, it frees member advocates to give fuller attention to each conversation. VytlOne earned a 4.89 out of 5.0 member-satisfaction score across more than 50,000 surveys in 2025, evidence that pairing AI with skilled people supports high-touch, personalized service rather than replacing it.

Is AI in pharmacy contact centers HIPAA-compliant?

It can be, and it should be. Any AI used in a pharmacy contact center must handle protected health information under HIPAA safeguards, with appropriate access controls, encryption, and vendor agreements in place. At VytlOne, responsible AI also means human oversight on consequential decisions and transparency about the technology’s limits. Member privacy and safety come first, and the technology is held to the same standard of care as the people who use it.

How does VytlOne approach AI governance in healthcare?

VytlOne treats AI governance in healthcare as a leadership responsibility, not a technical afterthought. Every AI tool we use goes through review for HIPAA compliance, bias testing, and human oversight before it reaches a member conversation.

Join us for the latest in our series, AI in healthcare. Learn how AI is transforming the way we track, predict, and promote medication adherence.

What is AI medication adherence?

AI medication adherence is the use of predictive models, smart devices, conversational AI, and drug-device combinations to help patients take their medications as prescribed. Machine learning identifies patients at risk of missing doses 7 to 30 days in advance. Connected pill bottles, mobile apps, and chatbots provide reminders, track usage, and surface adherence patterns pharmacists can act on.

Medication adherence, or the extent to which patients take their medications as prescribed, is a critical factor in managing chronic conditions. For treatment to be effective, patients need to take their medications at least 80% of the time. Overall, patient adherence rates are estimated to be around 50%. That means 50% of patients are non-adherent.

The consequences of non-adherence are significant. It can lead to poor health outcomes, shorter lifespans, increased hospitalizations, and more than $500 billion in avoidable healthcare costs each year linked to nonoptimized medication therapy. In the United States alone, approximately 125,000 deaths a year are linked to medication non-adherence.

How can we improve these outcomes? Artificial intelligence (AI) is emerging as a powerful tool to help patients stay on track. Here are five ways AI is making a difference:

What predictive models can actually do

Machine-learning models that combine pharmacy claims, electronic health records (EHR) data, connected-device signals, and social determinants of health can predict 7- to 30-day medication non-adherence with area under the curve values of 0.75 to 0.85. Pharmacists use the risk scores to prioritize outreach to patients who may be most likely to drop off therapy before they actually do.

These models have been validated in chronic conditions like type 2 diabetes and opioid use disorder, with similar accuracy across hypertension, heart failure, and oncology. The risk scores let pharmacy teams move from retrospective reporting to proactive intervention, identifying patients days or weeks before a missed refill would have flagged them. That shift from hindsight to action is the same one AI-driven pharmacy analytics brings to health system leadership.

Source: Solomon B. AI-Enabled Medication Adherence and Scalable Pharmacy Transformation. US Pharmacist. March 2026.

How smart pill bottles track adherence

Electronic pill bottles record dosing events in real time, prompt patients about missed doses, and surface the data to the pharmacy team. Health-system specialty pharmacies and contract pharmacy programs deploy these for patients on high-cost or high-risk regimens when a missed dose has clinical consequences and the cost of intervention runs well below the cost of a hospital admission.

The evidence base for the approach is strong. In a randomized, virtual clinical trial of patients with multiple sclerosis, those who received pill-bottle reminders showed better adherence and were more likely to take their medication on time than those who did not (Rice et al). A study of breast cancer survivors found a 97.3% adherence rate among those who received reminders compared with 88.3% among those who did not (Park et al).

Smart pill bottles work best when pharmacists set the escalation logic and follow up on the patterns the device flags. The bottle generates the signal. The pharmacist closes the loop.

Medication reminder apps and behavior tracking

Mobile apps designed for medication adherence prompt patients to take doses, log usage, and surface adherence trends the pharmacy team can review between visits. Health systems and specialty pharmacies use these apps inside post-discharge protocols and complex-therapy onboarding programs where patient persistence in the first 30, 60, and 90 days is the outcome that matters most. Adherence is one piece of a larger arc; see how AI shapes the specialty pharmacy patient journey from referral through refills.

A systematic review of mobile-app interventions found that consistent engagement with reminder apps correlates with better adherence behavior across chronic conditions (Perez-Jover et al). Roughly two-thirds of U.S. consumers have used a health app in the past year, which gives the pharmacy team a credible delivery channel for patients who already manage other health information on their phones.

The investment trade-off for health systems is the cost of app onboarding and ongoing engagement versus the cost of avoidable readmissions and lost therapy persistence. For specialty pharmacies in particular, app-based adherence support pairs well with the smart-bottle and predictive-model layers already in place.

AI chatbots for patient support

Conversational AI in pharmacy contact centers and specialty enrollment teams handles routine questions, surfaces patterns across calls, and frees the pharmacy team to focus on the harder conversations that need human judgment. The chatbots collect data on medication-taking habits, flag potential side effects or barriers to adherence, and route higher-risk conversations to a pharmacist or member advocate.

A systematic review of AI-based chatbots for promoting health behavior changes found consistent evidence that conversational AI can support adherence behaviors when paired with clinician follow-up (Aggarwal et al). The principle is the same in pharmacy: the chatbot scales the first layer of contact, but the pharmacist still owns the clinical decision and the relationship.

VytlOne’s own contact center is a working example of this approach. AI transcribes and analyzes member calls to surface adherence themes, deliver real-time coaching to member advocates, and prepare for AI-powered call summarization that reduces administrative load without reducing the human contact members value (see “Embracing AI Without Losing the Human Touch”).

FDA-approved smart drug-device combinations

Some adherence tools are built into the medication itself. The FDA approved its first digital drug-device combination nearly a decade ago: a tablet embedded with an ingestible sensor that confirmed each dose and transmitted the data to the provider. Smart insulin pens and connected inhalers followed, all designed to give clinicians real-time visibility into whether the patient is actually taking the drug as prescribed.

Health systems and specialty pharmacies managing chronic-disease programs use these combinations inside high-acuity workflows where the cost of non-adherence is measurable in hospital admissions and disease progression (Vallejos and Wu). For complex regimens with narrow therapeutic windows, the device data tells the pharmacist whether the next step is patient education, dose adjustment, or a different therapy entirely.

Smart drug-device combinations are not a fit for every patient or every medication. The selection criteria are clinical: the drug has to be costly enough or risky enough that real-time confirmation matters, and the patient has to consent to the data sharing. When those conditions hold, the device shortens the loop between dose and decision (Zijp et al).

How VytlAIQ supports medication adherence

VytlAIQ brings the mechanisms covered above into one operating layer for the pharmacy team. The platform’s predictive intelligence scores adherence risk using the same kinds of inputs (pharmacy claims, EHR data, connected-device signals, and social determinants) and surfaces the patients most likely to drop off therapy in the next 7 to 30 days. Pharmacists see the risk scores and the contributing factors inside their existing workflow, not buried in a separate dashboard.

The Liaison Intelligence module turns those risk scores into next-best-action recommendations for the pharmacy liaison or technician making the outreach call. Workflow automation handles documentation and routing, which frees the pharmacist to focus on the conversation that actually moves the patient. For health systems running AI specialty pharmacy or AI 340B compliance programs alongside adherence work, VytlAIQ keeps all three on the same data layer, so a flagged adherence risk on a high-cost specialty patient triggers the right escalation without anyone retyping data into a second system.

VytlAIQ is designed to amplify the pharmacist’s judgment, not replace it. The platform handles the data work. The pharmacist handles the decision.

Risks and limits of AI in medication adherence

AI in medication adherence is not a finished technology, and treating it like one creates real risk. Three areas deserve specific attention from health-system buyers.

Model bias. Predictive adherence models underperform in patient populations they were not adequately trained on, including older adults with cognitive impairment, patients with limited digital literacy, and patients facing language or affordability barriers. Subgroup validation, fairness testing, and drift monitoring are baseline requirements, not optional features (Solomon).

Data privacy. AI tools handle protected health information at scale. HIPAA compliance is necessary but not sufficient. Health systems should also evaluate the data-sharing posture of the vendor, the audit logging available to the buyer, and the governance over which decisions the AI supports versus which decisions stay with the pharmacist.

Replacement risk. AI does not replace the pharmacist conversation. Risk scores are decision support. The pharmacist owns the clinical decision, the patient relationship, and the judgment calls AI cannot make, especially in the patient populations where the models are least reliable.

Final thoughts

Improving medication adherence benefits everyone. It leads to better patient outcomes, fewer hospitalizations, and significant cost savings. With AI, we have new tools to identify, support, and engage patients in ways that make a real impact.

Frequently asked questions about AI medication adherence

How does AI improve medication adherence?

AI improves medication adherence by predicting which patients are most likely to miss doses, surfacing those patterns to the pharmacy team in real time, and routing the right intervention to the right patient at the right moment. Machine-learning models combine pharmacy claims, EHR data, connected-device signals, and social determinants to flag 7- to 30-day non-adherence risk with AUC values of 0.75 to 0.85. Pharmacists then prioritize outreach to the highest-risk patients before a missed refill would have caught the problem. The result is earlier intervention, better therapy persistence, and fewer avoidable hospitalizations.

What is AI-powered medication adherence?

AI-powered medication adherence is the use of predictive models, smart devices, conversational AI, and drug-device combinations to help patients take their medications as prescribed. The technology layer collects dosing events, behavioral signals, and physiologic data, then surfaces patterns that the pharmacy team can act on. Smart pill bottles record when each dose is taken. Wearables capture sleep, mobility, and other context. Machine-learning models combine those inputs with EHR and claims data to predict short-term non-adherence risk. The pharmacist interprets the signal and decides on the intervention. The technology scales the data work. The pharmacist owns the clinical judgment.

Is AI-powered medication adherence HIPAA-compliant?

AI-powered medication adherence tools handle protected health information, so HIPAA compliance is a baseline requirement for any platform a health system or specialty pharmacy deploys. Compliant tools encrypt data at rest and in transit, restrict access through role-based controls, log every data access for audit, and route patient information only through Business Associate Agreement-covered systems. The selection criteria for buyers should also include subgroup performance testing for bias, transparency about model inputs, and clear governance over which decisions the AI supports versus which decisions stay with the pharmacist. Compliance is necessary but not sufficient.

How should a health system get started with AI for medication adherence?

The most validated and operationally scalable entry point for pharmacy AI is medication adherence. A practical sequence starts with stabilizing the medication regimen for the highest-risk patient population, then layering in a smart pill bottle or predictive model where the cost of non-adherence is highest, usually in specialty pharmacy or chronic-disease programs. Track outcomes such as therapy persistence, hospitalization rate, and pharmacist intervention volume. Once those wins are documented, scale to additional patient populations and additional adherence layers. Governance, workflow integration, and pharmacist training matter at every step.

Where does the pharmacist fit when AI handles adherence monitoring?

The pharmacist owns the clinical decision and the patient relationship. AI handles the data work that pharmacists do not have the time to do at scale: scoring risk across thousands of patients, surfacing patterns across calls and devices, automating documentation and routing. The pharmacist interprets the signal in context, makes the call on the intervention, and follows up with the patient. Risk scores are decision support, not decision replacement. Predictive models underperform in some patient populations, including those with cognitive impairment or limited digital literacy, so the pharmacist’s judgment matters most in exactly the cases where AI is least reliable.

Machine-learning models combine pharmacy claims, EHR data, connected-device signals, and social determinants to flag 7 to 30 day non-adherence risk with AUC values of 0.75 to 0.85. Pharmacists then prioritize outreach to the highest-risk patients before a missed refill would have caught the problem. The result is earlier intervention, better therapy persistence, and fewer avoidable hospitalizations."

The technology layer collects dosing events, behavioral signals, and physiologic data, then surfaces patterns the pharmacy team can act on. The pharmacist owns the clinical judgment."

AI-powered medication adherence works at a layer above that. It predicts which patients are at risk of falling off therapy days or weeks before a missed dose would confirm it, routes the right intervention to the pharmacy team, and adapts based on how the patient actually responds. Reminders are reactive. Predictive AI is proactive. Both have a role. They solve different problems."

Compliance is necessary but not sufficient."

Governance, workflow integration, and pharmacist training matter at every step."

Predictive models underperform in some patient populations, including those with cognitive impairment or limited digital literacy, so the pharmacist's judgment matters most in exactly the cases where AI is least reliable."

What is AI-driven 340B program compliance?

AI-driven 340B program compliance is the use of artificial intelligence to keep a health system’s 340B program accurate, audit-ready, and compliant. It covers automated claim auditing, referral capture, eligibility detection, and continuous compliance monitoring, applying software to review every claim, flag risk, and surface issues that manual, sample-based review can miss.

Health systems run 340B programs under strict compliance rules and rising scrutiny. Artificial intelligence can help teams stay audit-ready while capturing more of the savings they are entitled to. This guide covers how AI applies to 340B program compliance, where it helps most, and where human judgment still has to lead; for program basics, see our complete guide to the 340B Drug Pricing Program.

How does AI improve 340B program compliance monitoring?

Health systems managing 340B programs face the same recurring obstacles: compliance monitoring, claim matching, and inventory management. AI tools automate repetitive tasks and reduce human error by reviewing data in real time. Practical applications include predictive inventory replenishment for entity-owned pharmacies and automated claims matching that checks purchases against Health Resources and Services Administration (HRSA) audit requirements. Because HRSA audits only about 200 covered entities a year and most finalized audits surface at least one compliance problem, software that flags duplicate discounts, diversion, and reporting gaps early is a meaningful safeguard.

340B by the numbers

$81.4 billion in covered outpatient drugs was purchased through the 340B program in 2024, up about 23% from roughly $66 billion in 2023. (HRSA; 340B Report)

~200 entities/year is HRSA’s audit cap, about 0.33% of all covered entities in 2024. (ADVI analysis of HRSA data)

63% of HRSA’s finalized FY2023 audits found at least one adverse finding, most often duplicate discounts, diversion, or reporting errors. (ADVI analysis of HRSA data)

20–50% is the typical range of 340B discounts on covered outpatient drugs. (GAO, via 340B Health)

How does AI support contract pharmacy compliance?

Machine learning models analyze transaction data across multiple contract pharmacies to find revenue-capture opportunities and compliance gaps. Those insights help health systems see which sites underperform, refine pricing and dispensing strategies, and coordinate entity-owned and contract pharmacy operations. For programs that rely on a wide contract pharmacy network, this is often where the largest share of missed 340B value hides the 340B revenue most systems miss“.

Partnering with a 340B pharmacy services expert

Pharmacy services partners bring the expertise and infrastructure that make AI adoption practical. The right partner helps you identify tools aligned with your program goals, such as 340B compliance software that monitors contract pharmacy activity and flags discrepancies before they become audit findings. Look for a partner whose technology is purpose-built for 340B rather than retrofitted from a general analytics tool.

How to implement AI in your 340B program

Start with a pilot aimed at one clear problem. Begin with compliance automation for a single entity-owned pharmacy or a subset of contract pharmacies, monitor the results, gather feedback from your pharmacy and compliance teams, and confirm the tool integrates cleanly with your existing pharmacy management software and third-party platforms. From there, assess performance against the metrics that matter: compliance rates, error reduction, and financial outcomes. Refine and expand across more of your 340B program while keeping legal and compliance teams close, so growth stays aligned with HRSA expectations.

How VytlAIQ supports 340B program compliance

VytlAIQ is a pharmacy operating system built for providers to reduce time to therapy, improve quality of care, and unlock 340B value. For a 340B program manager, four capabilities matter most.

The 340B compliance engine audits 100% of claims for 340B eligibility, surfaces missed opportunities, and flags compliance risk, so review is no longer limited to the sample a team has time to check by hand.

Referral capture retrospectively identifies missed 340B-eligible prescriptions and verifies compliance, recovering value that would otherwise slip away when a prescription is filled outside the captured network.

The Liaison Intelligence platform gives pharmacy liaisons real-time, rule-based recommendations to improve medication adherence and reduce time to therapy, connecting compliance work to patient outcomes.

The client portal and executive visibility layer puts dashboards and natural-language queries in front of CFOs, pharmacy directors, and clinical leaders, so the people accountable for 340B can see performance without waiting on a report.

Together, these capabilities move a program from periodic, manual review toward continuous, software-supported oversight, the foundation of real 340B optimization. Learn more about VytlAIQ.

Risks and compliance limits of AI in 340B

AI strengthens a 340B program, but it does not run one on its own. Software cannot make the HRSA-compliant policy decisions that govern eligibility, and it is not a substitute for a covered entity’s own compliance judgment. Claim-eligibility models can carry bias or mishandle edge cases, so any claim an AI flags as eligible still needs human compliance review before it drives a purchase or a repayment. Models are only as sound as the data behind them, which makes clean, well-governed data and clear audit trails essential. And because 340B data includes protected health information, any AI tool must handle it under Health Insurance Portability and Accountability Act (HIPAA) safeguards, with appropriate access controls and vendor agreements. The goal is a system where AI does the heavy, repetitive analysis and trained people stay accountable for every consequential decision. For more on governing these tools, read Responsible AI in 340B.

Final thoughts

Used strategically, AI makes a 340B program more efficient across entity-owned pharmacies, contract pharmacies, and the partnerships that support them. Thoughtful implementation and continuous evaluation are what turn that potential into measurable, long-term gains in compliance, savings, and operational performance. The health systems that win with 340B will be the ones that pair capable technology with accountable people.

Frequently asked questions

How does AI help with 340B?

AI helps 340B programs by automating the repetitive, error-prone work of compliance monitoring, claim matching, and inventory management. Instead of checking a sample of claims by hand, a covered entity can use software to review every claim for eligibility, flag duplicate discounts or diversion risk, and surface missed savings. AI can also predict inventory needs and analyze contract pharmacy performance. The result is a program that captures more of the savings it is entitled to while staying ready for a HRSA audit.

How does AI improve 340B program compliance?

AI improves 340B program compliance by finding risk and value that manual review misses. Machine learning models analyze purchasing and dispensing data across entity-owned and contract pharmacies to identify compliance gaps, underperforming sites, and missed 340B-eligible prescriptions. Automated claim auditing reviews every claim rather than a sample, and continuous monitoring flags duplicate discounts, diversion, and reporting errors before they become audit findings. The result is a program that stays audit-ready while capturing more of the value it is entitled to.

Does AI integrate with existing 340B vendors and TPAs?

Good AI tools are built to work alongside the systems a 340B program already runs, including its pharmacy management software, third-party administrators (TPAs), and contract pharmacy platforms. Integration quality varies, so confirm it during evaluation. Ask how the tool ingests electronic medical record (EMR), TPA, and contract pharmacy data, how it handles data governance, and whether it complements or duplicates your split-billing software. Start with a pilot that connects to one data source, verify the integration works cleanly, then expand.

Does AI-driven 340B compliance satisfy HRSA audit expectations?

AI can strengthen HRSA audit readiness, but it does not replace the covered entity’s compliance obligations. HRSA audits about 200 covered entities a year, and most finalized fiscal year 2023 audits surfaced at least one adverse finding, often duplicate discounts, diversion, or reporting errors. AI helps by monitoring claims continuously, flagging those risks early, and maintaining clear audit trails. But a person on the compliance team still has to review flagged claims and make the final call. The strongest posture pairs automated monitoring with human oversight and close work with legal and compliance.

How should a health system start using AI in its 340B program?

Start small and specific. Choose one clear problem, such as compliance monitoring for a single entity-owned pharmacy or a subset of contract pharmacies, and run a pilot. Monitor results against defined metrics like compliance rate, error reduction, and captured savings, and gather feedback from pharmacy and compliance staff. Confirm the tool integrates with your existing systems and handles data under HIPAA safeguards. Once the pilot proves value, expand across more of your program while keeping compliance teams involved at every step.
